We proudly serve our community’s rehabilitation services along with long-term care needs. Chestnut Ridge Nursing & Rehabilitation Center is unique in that they offer 24-hour, full-service sub-acute, skilled nursing, and long-term care, as well as specialized orthopedic, cardiac and neurological rehabilitation programs.

A caring group of professional staff members comprised of RNs, LPNs, CNAs, as well as Physical, Occupational and Speech Therapists, all work under the direction of your own physician. Our staff members are dedicated professionals who treat all patients and residents with compassion and dignity in helping maintain physical function, self-esteem and emotional well-being. We will make every effort to maximize each patient’s quality of life.

Chestnut Ridge Nursing & Rehabilitation Center has been serving the health care needs of Forsyth County seniors for more than five decades. Rooted in a tradition of excellence in long-term nursing care, our extensive experience enables us to better respond to the unique challenges of geriatric rehab and enhances our ability to restore patients to the highest levels of form, function and independence.

Chestnut Ridge Nursing & Rehabilitation Center, LLC is both Medicare and Medicaid certified, and participates in several managed care plans allowing us to assist you with most situations.
